Special Education Forms

IDEA and the PA special education school code require schools to plan for a student's transition from public education to adulthood beginning when a student turns 14 years of age. The parent student assessment form is designed to assist parents and students begin the discussion about goals for adulthood. This form should be completed by the family and returned to your student s teacher. The teacher will make a copy of the form for the family to refer back to throughout the transition process. The information from this form will be used to develop a formal transition plan in the student s individualized education plan (IEP).
