Volunteer Background Checks and Clearances
1, 2, and 3(a) or 3(b)[1],[2]
1. Pennsylvania Criminal History Background Check
Pennsylvania Access To Criminal History; (click “New Record Check” - Volunteers Only)
AND
2. Pennsylvania Child Abuse History Clearance Pennsylvania Child Welfare Information Solution; (log in to Child Welfare Portal and access your clearance or create a new account and apply for a clearance)
AND
3. FBI Criminal History Record (fingerprinting) or Volunteer Disclosure Statement
(a) If you have not lived in Pennsylvania continuously for more than 10 years, then you must complete an FBI Criminal History Record through Pennsylvania Department of Education, by clicking through:
IdentoGO (to access Identogo and begin application using service code below)
PA Service Code PDE Volunteer (for service code)
FBI Criminal History certifications obtained through the Department of Human Services will also be accepted by the District.
or
(b) If you have lived in Pennsylvania continuously for more than 10 years, then you must complete a Volunteer Disclosure Statement, available at:

The results of the record check will be made available to you and must be submitted to the District with this Application. (Or, a copy of the statement must be submitted to the District along with this Application.)
The results of the checks will be made available to you and must be submitted to the District with your Volunteer Application.
*Notes*
Child abuse history and/or criminal history certifications obtained for other volunteer activities may be submitted to the District as long as the certifications were obtained within the previous 60 months.
Certifications and checks must be updated every 60 months, along with an updated Application.
[1] The Act 34 and 151 documents are free of charge. There is no cost to submit the Volunteer affidavit. However, there is a small fee to apply for an FBI Criminal History Record, which is set by the federal government.
